The form is the programme
The QA form is the single biggest design decision in a quality programme. Most forms get added to over time rather than designed deliberately, and within two years they’ve grown to twenty items, half of which nobody can defend. The form that survives that drift — or the one that’s rebuilt to reverse it — is short, items earn their place, and the scoring is something evaluators can do consistently and agents can take seriously.
Items that drive outcomes vs items that fill the page
Every form item should pass two tests: does it map to a customer outcome, and can it be scored consistently? Items that pass both stay. Items that fail either come off.
Items that earn their place: the customer issue was resolved (FCR). The customer was understood and the response addressed their actual concern. The next-best-action was correctly identified. Compliance with regulatory or risk-critical items was met. The communication was clear and appropriately empathetic.
Items that quietly destroy QA value: "agent said their name within 5 seconds." "Agent used the customer’s name three times." "Agent followed the call structure exactly." Scripted-language adherence that fights customer rapport. Subjective tone scoring without calibrated guidance. Process adherence to steps that no longer drive outcomes.
The four categories that work
1. Customer outcome. The most important category and usually the most under-weighted. Did the contact resolve the issue, leave the customer satisfied, and avoid the need to recontact? Operations that lift the weighting on outcome see scores drift slower and FCR rise.
2. Process adherence. Was the right pathway followed? Did the agent route correctly, identify the right product, surface the right information? Process items earn their place when they map to outcomes; they lose it when they enforce process for its own sake.
3. Compliance. Regulatory items with real consequences. In financial services, healthcare, utilities, and other regulated sectors these are non-negotiable. Keep them on the form, weight them appropriately, and don’t hide them in the same score as the developmental items.
4. Communication. Clarity, empathy, professionalism. The hardest to score consistently, the most important to calibrate. Without calibration, the communication score becomes evaluator-personality data rather than agent-behaviour data.
How to redesign a form that’s drifted
Three steps. Audit every item against the two tests — outcome map and scoring consistency. Remove half — the discipline is in the cutting, not the keeping. Re-weight the remainder so customer outcome is meaningfully heavier than the rest. Run the new form in parallel for four weeks before switching. Track whether scores correlate better with FCR, CSAT, and retention than they did before; if they don’t, the redesign isn’t finished.
Common pushback
Stakeholders will resist removing items. The pattern is consistent: every item has someone who lobbied for it years ago, and removing it feels like ignoring them. The counter is to invite them to defend the item against the two tests. If they can’t, the item comes off. If they can, it stays. The conversation is uncomfortable; the form is better.
